Accident Summary:

UH-1D, sn: 66-1098, was flying chalk five (5) in a flight of five (5) lift ships with a second flight of five (5) thirty to sixty seconds to the rear. The flights were in a left hand holding pattern at approximately 1200 feet MSL awaiting instructions to proceed to a pick up zone. UH-1D, sn: 66-1098 was seen to execute a flare, yawed to the right, then began a descending left spin. The helicopter made almost four (4) 360 degree turns prior to impact. Something was seen to fly out or off the helicopter to its rear approximately when it started descending. During the last one-third of the flight the rate of descent increased, the attitude was described from a 45 degree to a 90 degree nose down, the rate of spiral deminished, and the flight path was near a vertical descent. Impact was in a near level attitude at a relatively high rate of descent. The first witnesses on the scene stated that the body of the aircraft was intact noting specifically that the transmission was still in its normal position and that the windshields were not broken. The intensity of the fire precluded removal of the bodies until some time later. The fire destroyed most of the helicopter.
